Thanks again! The bushing is not seized to the shaft, rotates freely and has slight play. I’m guessing this is wear, and the resulting tilting of the pulley causes the belt delamination?

Agreed. Stainless steel hose clamps would be a good upgrade, given the failure of the factory clamp.

And here's a pic of the 15 year old snubber ring that came out of it. I'm wishing I kept the old transmission for disassembly...the failure-prone center drive gear is available on eBay for $12.
