The hardest part in adding an ice maker to a refrigerator would be drilling holes for the wiring and water lines. You can easily drill into a refrigerant line and be sol in a hurry.

A friend of mine added an ice maker to a small chest freezer. On the third try he got it. The other freezers died during the attempt.
 
No, he bought all three with the intention of installing the ice maker in them. The first one he hit a line. He measured on the second one to avoid the line hit had hit on the first, but the lines were in a different spot. He did get a whole drilled in the third.

I just mention this since you need to be careful when modifying something vintage. It wasn't that sad to kill two chinese freezers, but a nice old fridge would be another story.
